Pilates with Stefanie welcomes you to participate in instructor led activity sessions with the understanding that you agree to the following terms. By signing this you agree: 

ACTIVITIES

I, {name}, agree to engage in this exercise or exercise program at my own risk. I am voluntarily participating in these activities, and assume all risk of injury to myself. I understand that I am in control of, and choose, my activities at all times and should modify, rest, or stop activities if needed. Pilates with Stefanie is not responsible for my physical health before, during, or after my participation in these activities. 

FACILITIES & EQUIPMENT

I agree to use the facilities and any equipment in the manner intended and follow all instructions provided by the instructors or staff. Failure to do so may increase the risk of injury. I agree to comply with all posted and stated rules regarding the use of the facilities and equipment. I agree to leave the studio and equipment in the same condition I found it, minus normal use. Misuse of equipment or intentional damage to facilities or equipment can be grounds for forfeiture of membership privileges, at the discretion of Pilates with Stefanie management. 

There is no gum, food, or beverages other than water allowed in the studio unless explicitly allowed by the instructor. 

Clothing with external brads, zippers, protruding buttons, or other such artifacts can damage the equipment or present a safety issues. Extremely loose fitting clothing can similarly present a safety issue. And clothing that does not adequately conceal certain body parts may be offensive to others in the class. I understand that if my clothing is deemed inappropriate or unsafe for these reasons I may be redirected to an alternative, safer workout option and I will comply with instructor requests. I will not be denied a workout but it may be modified if needed. Pilates with Stefanie encourages everyone to wear comfortable clothing that facilitates a fun, safe workout for all. 

I also acknowledge there is no landline telephone on the premises and members and instructors are encouraged to have a cell phone available to use for emergencies, if needed. 

HEALTH

I agree not to attend classes or visit the studio if I have a fever or any other indication that I have a contagious condition.

I agree to clean the equipment after I have used it with the cleaning supplies provided by Pilates with Stefanie

INCLUSIVITY

I understand that Pilates with Stefanie is an inclusive environment and I agree to treat staff, instructors, and fellow participants with courtesy and respect. Harassment of staff or participants may be grounds for forfeiture of my membership, at the discretion of Pilates with Stefanie management. 

SIGNATURE

I acknowledge that I am signing this agreement freely and voluntarily and intend my signature to be a complete and unconditional release of liability to the greatest extent allowed by law.
